Incoming Grade 7

Grade 7 applicant should submit the following requirements to the Admission Office:

• • • • • ENTRANCE Exam

        • 2 Photocopy of PSA Birth Certificate

        • 3 pieces 2" x 2" recent colored ID Picture 

        • Copy of the Report Card w/ LRN (1 Original and 2 Photocopies) duly signed by the school principal.

        • Certificate of Good Moral Character from the Guidance Counselor or School Principal.

        • Recommendation Form (placed in a sealed envelope)

        • Baptismal Certificate/Certificate of Dedication (for non-Catholic)

        • Marriage Certificate (if Parent are married)- OPTIONAL

        • 2 pieces Long Folder

        • 1 Long Brown Envelope

*All documents should be placed in a long brown envelope labeled with name written in pencil.
